75yr hypertensive, cardiac , with prev4 cs all midline , last with herniorraphy with 3 ant. abd wall incisional hernias one supraumb. hernia 1 infra umb. small hernia & a sliding hernia with a very wide base & broad lig. hernia (quain hernia) .. complaint with abd enlargement for 1 year she was diagnosed before as Mucinous cystad. & offered surgery but she refused ! up until she's had repeated attacks of intestinal obstruction she presented with severe dehydration & IO exploration surgical staging with just nodal sampling and infracolic omentectomy to reduce morbidity then onlay large mesh pathology granulosa cell tumour stage 1a postop recovery was very slow & she had an attack of ileus that resolved with conservative management surgical team : working with my dearest prof Sayed shaker surgical oncologist Anasthesia team led by Dr Haitham Hassan
