The Crash: Flight 268 departed Karachi at 11:13am. Upon contacting Nepal ATC, the aircraft was cleared for the Sierra approach, which the plane was to descend in seven steps to 5,800 feet, in order to allow the flight to safely pass over the Mahabharat Range, south of Kathmandu. At 2:30pm local time, the aircraft crashed and disintegrated on impact at 7,300 feet onto the side of a mountain near the village of Bhattedanda, killing all on board. The investigation revealed that the flight had made its decent one step too early. Due to this, every step the flight then took, it was below its assigned altitude, which inevitably caused it to crash into the mountain. This error was enhanced due to poor visibility and that the ground proximity warning would not have been triggered due to the steep terrain. The approach plates for Kathmandu issued to PIA pilots were also determined to be unclear and that ATC in Nepal were not trained properly to react to such situations. The crash happened just 59 days after Thai Airways Flight 311, which also crashed into the side of a mountain near Kathmandu. This crash is also the deadliest to occur on Nepalese soil.
