John Gibbons is a sports Osteopath and a lecturer for the 'Bodymaster Method ®' and in this video he is discussing the origin, insertion and action of the Tensor fascia later (TFL) and the iliotibial (IT) band muscle of the hip joint. The TFL attaches inferior and lateral to the ASIS (anterior superior iliac spine) and travels via the IT band or IT tract to the lateral tibial condyle and is called Gerdy's tubercle as named after the French Surgeon - Nicholas Gerdy. The TFL is an hip flexor working with the psoas, iliacus, rectus femoris, adductors, and sartorius. This muscle also abducts the hip working with the Gluteus Medius and Gluteal Minimus. This muscle is innervated by the Superior Gluteal Nerve from L4 and L5 of the lumbar-sacral plexus. The TFL and especially the IT band runs across the lateral femoral condyle and this irritation is called runners knee or specifically iliotibial band friction syndrome. This condition can also affect the tracking of the patella and cause patella femoral pain syndrome as well. The Obers test and Noble tests are commonly used to ascertain dysfunction with this tissue. John also discusses the Rectus femoris of the quadricep group that originates from the AIIS of the anterior pelvis and attaches via the patella ligament to the tibial tuberosity. It is an extensor of the knee and a flexor of the hip and is innervated buy the femoral nerve from L2,3,4 of the Lumbar Plexus. If this muscle is tight it can pull the pelvis anteriorly and subsequently causing the lumbar lordosis to increase causing facet joint - back pain. Other knee pain is from the the cruciate ligaments especially the Anterior Cruciate Ligament tears and the Lachmans or Drawer test is used. MCL tears of the medial collateral ligament are very common, osgood schlatters that affect the teenager as well chondro malacia patella (CMP) cause a lot of pain.
