La trama è impostata tre anni dopo Dead Space: Extraction. Gabe Weller, che ora lavora per la Sprawl Security, sta facendo un giro di pattuglia nelle miniere di Titan, quando si scatena l'invasione di necromorfi. Gabe, preoccupato per la moglie che si trova sullo Sprawl, decide di raggiungerla e portala in salvo insieme al bambino che ha in grembo. Il DLC si svolge parallelamente alle vicende di Dead Space 2; infatti, Gabe attraverserà molte aree già attraversate da Isaac Clarke.
