Before you even go to Baghead you will need to obtain some moonshine, a loot, and a Roasted Rat. Once you've fought your way to Baghead you will give him the Roasted Rat first. Then interact with him again to give him the Moonshine. Interact with him 1 more time while having the loot and you will play a whole ending scene with him and obtain this trophy / achievement.
