This is the ending where only Joker and Traynor survive, genophage cure sabotaged, chose geth over quarians, and proceed to Destroy the Reapers (which also killed all synthetics, including EDI and the Geth). There was a bug in the original where the nameplate floats by its own on the way to the memorial wall. Glad they fixed it in the Legendary Edition. Personally, achieving this ending was extremely painful. Not only you have to lose all your beloved squadmates (yes, even Garrus and Liara), but you also have to do war crimes to 4 entire species in the very same game.
