It can also be micro tears or over stretching of the muscle. It can show itself with pain, swelling or tightness. In Grade 2, more muscle fibers are damaged. The tendon is still attached to the bone but heavily damaged. Besides the swelling, bruising can occur because of hematoma. And the most severe one is Grade 3. Muscle is either completely detached from the bone or a very small piece of it is attached to the bone. In muscle groups like pecs which have two heads, rupture of one head can be named as Grade 3, too. For example rupture of the sternal head while the clavicular head is still attached is Grade 3, too. If you are experiencing this kind of tear, you will probably notice that. A sound - yes you will hear it tearing -, a sharp sudden pain, and even sometimes a shoulder dislocation can come along. Right after that, swelling and bruising meaning hematoma all over your shoulder, chest and arm can be seen. First of all, it doesn't matter which Grade is your tear and what kind of treatment, conservative or surgical will be done, first intervention after the injury is very important. There is a protocol called RICE that is applied after these kind of injuries. RICE stands for rest, ice, compression, elevation. Rest is important for preventing the tear from going worse. Because, for example a Grade 1 tear, as the area is already damaged, if worked again without resting, is a candidate for a Grade 2 tear. Also, a detached muscle, meaning Grade 3 will get shorter if you overload it. And this will cause the tendon to retract away from the bone and cause a future operation to be harder. So, when you feel that you experience a tendon injury, start the treatment immediately with RICE protocol. You can even put your arm in a sling to minimize the movement in pec tears. And go see a doc who is an expert on sport injuries as soon as possible. Stuff like stretching the muscle, keeping the area warm or letting it rest to heal and doing nothing are not the right approaches. By the way, when it is a pec rupture, make sure the doctor is experienced in shoulder injuries. The treatment will not have the same result with any orthopedic surgeon. Which treatment method should we choose then? For Grade 1 tears, RICE and simple physical threapy methods will be enough. You will probably get back to sports and activities in 3 to 4 weeks of rest. Grade 2 tears are in a gray area. Usually a longer resting period and a more advanced physical threapy than Grade 1 tears would be seen enough but a surgery may be necessary for these type of tears, too. This is a decision that you will give with your doctor. As I mentioned in the first video, Grade 3 tears should be treated with surgery IMHO. At this point, let's talk about surgical methods. Actually the surgical procedures that are applied to Pec tears look very simple on paper. It's all about stitching the tendon which was separated from the bone, back to its normal connection point. To be more precise, first of all they make a clear cut to the arm-length, at the exact point where they are going to connect the tendon. From this point, they get in and find the terminus of the damaged tendon. At the same time, they drill the humerus which they are going to connect to the tendon. And insert small anchors over it. Mostly, they stich the tendon to this anchors with titanium strings. There may be some factors which can complicate this for sure. First of all if it's a delayed operation, the torn muscle which has retracted in the middle of the chest is going to loose its flexibility. If the tendon has been retracted from the nipple in to the middle, holding it and stretching out all along to the bone may not be possible. In such circumstances, using an allograft may be necessary. An allograft may be an Achilles' tendon which has been taken from a cadaver. Or a piece of the hamstring tendon of the patients own. In such cases, the success of the operation and the cosmetic expectations can be lowered. At this point, I must repeat once more that an early operation for this kind of injuries is crucial. A part from that, because the point which they make the cut is close to the arm pit, the risk for a deep infection may be higher. Because the muscle has failed to act, there may be a fatty degeneration and that may cause a loss of function in the muscle.
